Charcot Joint Salvage

Charcot foot is a serious complication of diabetes, where there is progressive destruction of bones, joints, and soft tissues in the foot and ankle due to neuropathy (nerve damage).

Causes:

  • Peripheral neuropathy (loss of protective sensation)
  • Minor trauma or repetitive stress goes unnoticed
  • Continued walking leads to bone and joint breakdown

Symptoms:

  • Warm, swollen, red foot (often mistaken for infection)
  • Little or no pain despite severe damage
  • Foot deformity (e.g., “rocker-bottom” foot) in later stages

Diagnosis:

  • Clinical signs in a diabetic patient with neuropathy
  • X-ray, MRI, or bone scan to assess bone damage and rule out infection

Treatment:

  • Early (acute) phase: Immobilization with total contact casting, non-weight bearing
  • Later phase: Custom footwear or braces to support the foot
  • Surgery: Required for severe deformities or unstable joints
